For families and couples, if applicable, kindly complete this form multiple times, once for each individual. Kindly note that Magenta Health physicians are accepting patients on a limited basis, and may not be able to accommodate additional family members.

The next step is for us to validate your OHIP information and to send you more information by email. This email will also include a way to schedule your first appointment online.

It may take us up to a couple weeks to complete these administrative steps and to send you the invitation email. If you haven't heard from us after two weeks, please don't hesitate to get in touch with us by email at contact@magentahealth.ca to follow-up in respect of your submission.
