Last updated: May 1, 2024
Within the past 5 days…
Have you or any individual in your group had any of the following symptoms?
Fever and/or chills, cough, sore throat, shortness of breath, headache, decrease in sense of smell or taste, runny nose, or nasal congestion.
If you do not have any of the symptoms listed: You have screened NEGATIVE. You may continue with your in‑person appointment, and nothing further is needed from you.
During cold and flu season (~October - April), we strongly recommend that you bring a mask to wear within the clinic at all times to protect yourself, and vulnerable patients within our community.
If you do have any of the symptoms listed: You have screened POSITIVE. You may continue with your in-person appointment, but kindly:
bring a mask to wear within the clinic at all times, including when you are in the exam room.
complete a Rapid Antigen COVID-19 test prior to your appointment if possible (learn more about Rapid Antigen tests and where to find them here). Having the results from a COVID-19 test is helpful for seeking antiviral treatment (if eligible), and to ensure appropriate precautions are taken to keep the community, staff, and physicians safe.
