I would like to send my physician medical records

If you have a copy of your medical records:

If you have digital copies of your medical records (such as a PDF, a CD or a USB) you can send them to us electronically. If your file is larger than 20MB, please note you may need to split it into multiple parts.

Alternatively, if you have a physical copy - or are unable to use the tool above - the physical copy, CD, or USB can be either (1) dropped off at your primary clinic location during our regular business hours, or (2) mailed to us at any of the clinic addresses listed here.

If you Need to provide vaccine Records:

COVID Vaccinations: There is no need to provide us with these records. This information is automatically provided to us.

Any other vaccination provided elsewhere: If you aren’t sure if your records have been provided to us by the party that administered the vaccine, you can send them to us electronically, or request the third party fax them. Your physician’s fax number can be found here.

If you were seen by a specialist:

If you were referred to a specialist by a Magenta Health physician, it is very likely that they have forwarded us a copy of your report/consult/results via fax.

If you were seen in a walk-in clinic , IN the ER , or out of province:

The healthcare provider you saw may have asked for the information of your primary care physician so that they can send us the reports directly. In which case, there is no need for you to send additional copies.

The best way to forward your physician any reports is to ask the facility you visited to send it to us directly. Your physician’s fax number can be found here.
